IF YOU PLAN TO TRAVEL TO IRAN,DO NOT WORRY ABOUT YOUR MEDICAL COVERAGE


Dana Insurance Company through its direct contract with over 80 hospitals and medical centres across the country will cover, upon any unforeseen occurance of illness or accidents your medical requirements.

 

1. Payment of medical expenses.

2. Payment of emergency dental expenses.

3. Payment of medical Transportation (by air, land and/or sea ambulance) to hospital.

4. Legal assistance.

5. Assistance in case of any problem ( loss or missing travel documents).

6. Repatriation of any child under the age of fifteen ( in the case of death) or hospitalization of the insured.

7. Payment of one round-trip ticket for any one accompanying the insured .

8. Payment for the costs of repatriation of the deceased body.

* Groups between 10 to 20 will receive 5% discount.

ACTIONS TO BE TAKEN IN THE EVENT OF A CLAIM

On the occurance of an accident or other unforeseen events, the insured person is to contact the emergency calls centre of Dana Insurance Company in Tehran, Iran, telephone: (9821) 8791022 immediately, quoting the data on his certificate of insurance . If it is not possible to make such an emergency call prior to recieving the services of the doctor/hospital, the insured person is obliged to contact the emergency calls centre immediately after the start of treatment.



 

General Conditions


 

1.1 DEFINITIONS

As used herein, capitalised terms shall have the following respective meanings:

1- Insured : The person under 80 years of age, and covered under a valid Travel Insurance Policy issued by DANA Insurance Company.

2 - Medical Team : Medical structure appropriate to every Claim and defined by DANA Insurance Company chief medical officer and the treating physician.

3 - Bodily Injury : Wherever used therein means bodily injury caused solely and directly by violent, accidental, external and visible means, excluding illness or disease, and occurring after the beginning of the journey abroad undertaken by the Insured during which such bodily injury gives rise to a request for assistance by the Insured or his representatives.

4 - Illness : Wherever used therein means sudden and unforeseen sickness or disease contracted, commencing or originating after the beginning of the journey aborad undertaken by the Insured during which such sickness or disease gives rise to a request for assistance by the Insured or his representatives.

5 - Claim : Any event which may give rise to DANA Insurance Company interventions.

6 - Relative : Insured?s father, mother, child, spouse, brother or sister.

7 - Serious Medical Reason : Bodily Injury or illness of such gravity that it endangers the person?s life.

8 - $ U.S. : Shall mean the lawful currency of the United States of America.

9 - RLS : Shall mean the lawful currency of Islamic Republic of Iran.

10 - Deductible : part of the expenses born directly by the Insured.

1.2. TERRITORIALITY

The benifits mentioned in chapter II apply inside IRAN

1. 3. PERIOD OF COVER

Assistance will only be granted for travels inside Iran which do not exceed 92 consecutive day.

1.4. PRESCRIPTION

Every claim in respect of a covered event shall be time barred within one year from the date of occurrence of such event.

1. 5. SUBROGATION

Dana Insurance Company shall take the place of the Insured, to the extent of the cost incurred or indemnity paid, in the rights and actions of the Insured against anyone responsible for an accident causing a claim for one of the hereinlisted benefits. When the services provided by DANA Insurance Company are covered in whole or in part by another scheme. DANA Insurance Company shall be subrogated to the rights and causes of action of the Insureds against said schemes.

1.6. LIABILITY

DANA Insurance Company shall not be held responsible for delays or failures to provide the assistance in case of strike, war, invasion, act of foreign enemies, hostilities (whether war be declared or not), civil war, rebellion, insurrection, terrorism or military or usurped power, riot and civil commotion, radioactivity or any other event of force majeure



 

General Exclusions



 

 

A) Assistance will only be granted for travels no longer than 92 consecutive day.

B) The Insured will not be entitled to the reimbursement of the expenses directly paid by him or by any person on Insureds behalf without the previous declaration to and the authorisation of DANA Insurance Company Alarm Center as listed in Chap. IV

C) Furthermore are excluded the claims :

a) Resulting from travels with a therapeutic objective

b) Resulting from treatments provided in the country of residence

c) Resulting from prophylactic treatments or vaccinations

d) Resulting from spa cure, physiotheraphy, heliotherapy or aesthetic treatment

e) Arising out of any existing, recurring, chronic or continuing illness or any conditions of which the Insured is aware of or for which he received treatment. The convalescence is considered as a full part of the illness.

f) Resulting from war, invasion, acts of foreign enemies, hostilities (whether war be declared or not) civil war, rebellion, insurrection, terrorism or military or usurped power, riot and civil commotion.

b) Resulting from intentional self injury and from the participation of the Insured to criminal actions.

c) Occuring whilst the Insured is engaged as competitor in bets, horse races, bike races and any kind of motor races or exhibitions.

d) Due to the participation of the Insured to a fight except in case of self defense.

e) Due to the practice of sports, on a professional basis or in order to train for an official competition or an exhibition.

f) Directly or indirectly caused by or contributed to by or arising from ionising radiation or contamination by radioactivity from any irradiated nuclear fuel or the radioactive toxic explosive or other hazardous properties of any explosive nuclear assembly or nuclear component thereof.

g) Arising out of any chronic or continuing illness or condition of which the Insured is aware or for which he received treatment before the beginning of the period of coverage during which such illness gives rise to a request for assistance by the Insured or his representatives. The convalescence is considered as a full part of the illness.

h) Consequent upon pregnancy within three months of the expected date of delivery or upon voluntary abortion.

i)Caused by mental illness or alienation.

j) Sustained whilst under the influence of or disablement due wholly or partly to the effects of intoxicating liquor or drugs other than drugs taken in accordance with treatment prescribed and directed by a qualified registered medical practitioner.

k) Resulting from death, injury or illness due to suicide or attempt of same.

l) Whilst the Insured is engaged or taking part in racing, motor rallies and competitions, professional or organized sports, mountaineering (reasonally requiring the use of ropes or guides), pot holing, ski diving, winter sports, aeronautics or any form of serial flight except as a fare-paying passenger of a property licensed aircraft being operated by a licensed commercial air carrier or owned and operated by a commmercial concern.



 

Benefits



 

II.1. Medical Transportation

Should the Insured suffer accidental Bodily Injury or sudden illness (and/or compulsory quarantine), during the period of cover, DANA Insurance Company will organise and pay for :

(I) The admission of the Insured into the nearest hospital

(ii) If necessary on medical grounds, the transfer of the Insured by any means (including but not limited to air ambulance, scheduled flight, road ambulance...) to a more appropriate hospital according to the nature of the injuries or of the illness, and the repatriation to domicile whenafter local treatment the condition of the Insured will not prevent his repatriation to domicile as a regular passenger, according to the medical opinion of the treating physician and DANA Insurance Company Medical Team. This repatriation will be carried out by scheduled airline or other adequate means, should his original return ticket not be valid for that purpose.

 

Or,

 

(iii) Repatriation of the Insured by any means to a hospital near to his home in domicile, if his condition does not prevent such a repatriation.

11.2. Payment of medical expenses

100,000 Rls deductible shall apply in all cases on each Claim, except in case of Bodily Injury and/or hospitalization for at least 24 hours. DANA Insurance Company guarantees the payment of the emergency medical, surgical, pharmaceutical and hospital expenses incurred by the insured or any person acting on his behalf, on medical prescription, in urgent cases arising as a result of a Bodily Injury or a sudden Illness, with the exclusion of any kind of prosthesis.

In any case, the medical expenses are covered up to the time the Insured meets the medical conditions for a repatriation in Domicile, conditions which are assessed by DANA Insurance Company medical team and the treating physician. Should the Insured refuse the repatriation to home country, the medical expenses shall then be covered up to the amount of the costs of the repatriation, should this later have been performed.

11.3. Medical Referral

In an emergency situation, Dana Insurance Company will provide the Insured with basic information such as : names of doctors, specialists, dentists or paramedical staff nearby, location of hospitals, medical centres, drugstores, ambulances ... and give simple advice as to the action to be taken with the exclusion of any medical diagnosis.

11.4. Payment of emergency dental expenses

Subject to a limit of Rls. 500,000 per Insured and per period of cover.DANA Insurance Company guarantees payment of emergency dental expenses incurred by the Insured or any person acting on his behalf in emergency cases,arising as a result of a Bodily Injury or of an illness, with the exclusion of any of prosthesis and or definitive filling.

11.5. Repatriation in case of death

In the event of the death of the Insured DANA Insurance Company will take all the necessary measures to organise the repatriation of the body and will pay for the post mortem treatment and the basic coffin required for international transportation, and the transportation of the remains to the airport nearest to the place of burial in domicile. Funeral or burial expenses are not covered.

11.6. Legal assistance

DANA Insurance Company undertakes to organize and pay up to the equivalent amount in local currency for the legal defense of the Insured, should he/she be prosecuted for civil liability according to the Civil Code in force in the country further to accidental damage caused to a third party or involuntary violation or no respect of the laws and local settlements of administrative nature in a private life situation, as simple citizen, with exclusion of the claims arising out of the use, possession or safe-keeping of a motor vehicle.

11.7. Emergency visit

If the Insured should be hospitalised as a result of a Bodily Injury or illness for more than 10 (ten) consecutive day, DANA Insurance Company will, at the Insured?s request, provide and pay for one round-trip ticket (on scheduled flight economy class) for any one person designed by the Insured coming to his bedside.

11.8. Enquire in case of a problem

In case of loss or theft of essential travel documents such as passport, visa, airline ticket ... DANA Insurance Company will provide the Insured with the necessary information regarding the formalities to be fulfilled with the appropriate local authorities or entitles in order to obtain the replacement of such lost or stolen documents

11.9. Return of child(ren) left unattended

When an Insured has been evacuated and/or repatriated as per the assistance service II.1. ?Medical Transportation? or II.5. ?Repatriation in case of Death", and is no longer able to attend to any child under the age of fifteen who may have been travelling with him at the time of occurrence of the Assistance Event, DANA Insurance Company will organize and pay for the return of such child to domicile, under the supervision of an adult person.



 

OBLIGATIONS OF THE INSURED



 

 

A) GENERAL PROCEDURE



 

 

1) REQUEST FOR ASSISTANCE

In case of emergency, the Insured shall call, one of the following DANA Insurance Company V.I.P 24 hours a day Alarm Center :

Before undertaking any personal action and :

- state his name, the number and validity dates of his Travel Insurance Policy.

- state the place and telephone number where he/she can be reached

- give a brief description of the problem encountered and nature of help required

2. FAILURE TO NOTIFY DANA Insurance Company

DECLARATION

Any declaration of a claim shall be made within maximum 48 hours after the beginning of the hospitalisation or the medical consulting.All declaration made after this delay shall be deemed as nil and therefore be rejected by The Insurer.

Life threatening situation :

Any other of this Appendix or of This Agreement notwithstanding, in a life-threatening situation, the Insured or his representative should always try to arrange for emergency transfer to a hospital near the place of occurrence of such situation through the most appropriate means, and to call DANA Insurance Company V.I.P Centre to provide the appropriate information as soon as practicable.

Hospitalisation prior to notice DANA Insurance Company :

In any case of Illness or Bodily Injury requiring hospitalisation, the insured or any person acting on his behalf must inform DANA Insurance Company within 48 hours from the time of occurrence. Failure to do so may entitle DANA Insurance Company to invoice the supplementary cost to be borne by DANA Insurance Company which would not have been incurred had this 48 hours delay been respected the Insured.

 

B) MEDICAL TRANSFER

 

If the Insured claims for a medical transfer or repatriation, the following conditions have to be observed :

i) In order to enable DANA Insurance Company to take action as quickly as possible the insured or any person acting on his behalf must give:

- the name, address and phone number of the hospital where the insured has been taken

- the name, address and phone number of the treating physician, and if necessary the family doctor

ii) DANA Insuranc Company Medical Team or agents shall have free access to the Insured in order to ascertain his condition if this obligation is not fulfilled and except justified opposition, the Insured will no longer be entitled to medical assistance.

iii) In any case, DANA Insurance Company, upon agreement with the treating physician, will choose the date and means of transportation. Medical transfer or repatriation by air ambulance shall be limited to continental transportation.

iv) Should DANA Insurance Company pay for the transportation of the Insured, the latter will be asked to hand over to DANA Insurance Company the unused portion of his original ticket or its counter-value.

 

C) GENER?L PROVISIONS

 

- Limitation

As soon as a Claim occurs the Insured must make every endeavour to limit or stops its consequences.

- Recovery

The Insured will provide Dana Insurance Company with all documents and all necessary formalities so as to enable Dana Insurance Company to recover from relevant sources, if applicable.
